Below you'll find samples of email collections forms I have put together. The goal of all of them was not just to accomplish the task of collecting customer emails but also to brand the forms properly by finding appropriate images and fonts/typefaces.

This form is intended for a fun , casual company. Something like a fun gift shop. It was fun making it - especially finding the images for it.

The combination of a fun typeface and the appropriate images that would convey the thought behind each stage of the form were key to this example. Also notice the large size of this collection form. I felt this was appropriate and almost resembles the size of a children's book.

I'll use any excuse to use a dancing Carlton GIF in a design. Besides that, it was an experiment with using animated GIFs. This type of form would work best with a company that conveys a fun tone, not for something serious like a financial company or an architect.
